It depends on the ROM. With Mighty ROM it comes as an .exe that includes the RUU. Once you unlock you run the .exe and voila. You will then need to apply the Spint carrier cabs and install the MMS cab. Its all documented in the Mighty thread.

I'm on Sprint and just did this the other day and couldn't be happier. All my annoyances are fixed and everything is stable. Well worth the time spent.
