check to make sure you dont have a P.O. Box on your billing account. if you do then you may have to call the rebate number for status and possibly to send one out. If your adress is normal then head to vzw.com and type in "rebate" in the search at the top then choose the first option. from there choose "manage rebate submission" then choose "track your rebate" type your lastname and zip and any other information that you know. it will let you know if its been sent and if you havent gotten it call rebate number.
