OK, this is a long shot but go into S2U2 and turn ON BOTH haptic and vibrate on connect then SAVE. Exit out, if it doesnt do it for you. Go back in, turn them both OFF and then SAVE.
If that doesn't do it then I'd go with the Stock flash because from my end you're having conditions that I dont see in 920, but maybe it's ME who is the odd man out. Maybe someone else can confirm if those features are working in the latest build?
Edit:Found this-Start>System FOLDER>Phone Wizard>Scroll down there IS a Vibrate when call is connected but mine is on and my phone doesn't vibrate.

The first 9/20 build had haptic vibration turned on by default. NRG uploaded a second version where it was turned off.
