Im having the same issue, and im on the 9/6 Old Manilla, even thought i just use Titanium, the in call volume works with the HTC dialer, and it has a slightly different volume display when in call, it does not have a silent or vibrate button. When i disable the HTC dialer, and use standard, winmo dialer, when i attempt to adjust the volume while in call, does not adjust, i see the volume display change but its only changing the system and ringer volume, not in call volume. so im a little lost on how to fix it

EDIT Brett what i did to adjust the volume from blasting my ear drums is go to Start->Utilities->Tweaks->TouchPro Tweak-> in that app go to 3. Volume Control->3.4 Increase Sound-> set to disabled.

This lowers volume in call which made it more bearable.
