Welcome to PPCGeeks! This is what you must do:
1. Hard reset your device.
2. After WM6 boots up and asks you to initialize your screen and set the time zone, look for the countdown ("customizing your device in 3,2,1 seconds") and immediately interrupt the carrier customizations with a soft reset.
Your carrier's customizations are already built into the rom. Running them again from the extended rom screws up the registry and causes your symptoms among others.
