Uhm, I'm only an Internet lawyer... but I'm almost positive that if you're one of the participating parties that you're allowed to record the conversation (with or without conscent) if the recording is for personal or journalistic purposes.
If the recording is for anything commercial, recording must be disclosed (the reason why most companies say that this call is being recorded and "may" be used for training purposes.)

Depends on the specialist, and how life threatening it is. Elective surgery/specialists, the line can get pretty long 4 - 6 months isn't that uncommon. Either way, I'm quite confident that an extremely low percentage (like zero) of Canadians would be willing to give up their system for the US one.
