Re: should be so easy but......
Used to have a similar need, not for actual work, but for our office football pool. We used a Word form, but I doubt that'd function in Word Mobile. We now use the free feedback form available at freedback.com, works great but involves HTML and an outside server. Hmm, doesn't Adobe have a PDF authoring package that can create forms? It wouldn't be free, but should work for you.

Re: should be so easy but......
Found this:
As of LiveCycle ES 2.5, our Mobile applications for iPhone, Android, Blackberry and Windows Mobile are capable of rendering forms! In order to develop a "form" that will work on the mobile devices, you need to use LiveCycle Guilde Builder, which is... Here: Adobe Forums: LiveCycle ES2 Mobile Applications Wait, it also says: The mobile client will not work without a concurrent LiveCycle ES implementation.... Re: should be so easy but......
If your employer is not willing to pay for Adobe's mobile client implementation (see last link above), then freedback.com can solve your issue nicely. You can put freedback's code into your own HTML so your form can custom fit your PPC screen:
