the SIM unlock should survive a NV reflash. I ended up giving a spare NV to another user who had bricked their unlocked phone, and I know that my NV was on a locked device. Their device remained unlocked.

I think that the SIM unlock is written to the phone's radio firmware, and not to the NV. I've looked for a way to unlock it via NV, but there isn't any way to do it.
