Yes, you need an account with Sirius, just like any other satellite radio provider...this app just allows a registered user to listen via mobile.

I am a Sirius subscriber and the .cab is working after I downloaded Core Player. I then did not feel like paying for Core Player, uninstalled, and the Sirius application still worked using Windows Media player to listen to the music feed.
