Is this what I need to fill out and mail in to recieve me $100 rebate for the TP2? If it is not, can somebody hook me up with it. thanks!

I've used the print-out ones like that, and yes, they work. My check is due in 3 weeks. Just be sure you fall between the purchase dates. If not, you'll have to get the older form

but I can us that mail in rebate even as I got my phone as a replacement. what do you mean your check is due in 3 weeks? thats when your getting it?

make sure you make yourself a copy of your receipt in case the one you send in gets "lost".

If you got it at a sprint store... You can get them to print you out a copy if you don't have your original. I had to do that. lol.
I chuck all my receipts as soon as I get them. LOL.
