I havent had the phone for a year

I purhased it from ebay and use the sprint mogul on pageplus wireless
So would there be a possible chance that I could get this phone fixed/replaced under warranty somehow?

No. If it's ported to another carrier there is no warrenty.

What if you purchased the phone used and requested the insurance within the 30 days but they never added the insurance? That is my situation.

They will not put insurance on a used phone. It can transfer from a brand new phone to a used, but not added. I'll say it again, you're outa luck. Time for a new machine...
