Just a quick search and I found this:

Looks like its a SIM card issue. Hope that helps.

Re: puk code

I have looked all over the plastic card that the sim was actually cut out of and I looked at all the papers in the original box of my TP2 from Sprint and I don't see and sim unlock, or for that matter, any other kind of sim code numbers at all. I see numbers but none of them seem to be the PUK code I am looking for to clear the annoying screen that keeps popping up on my CDMA phone. I want to be able to find the PUK on my own without having to contact Sprint if at all possible. I wonder if I ran a sim security unlock on it if that would fix the problem and get rid of the PUK pop up screen?

Re: puk code

I've only ever seen PUK code issues when on a GSM provider (e.g. AT&T). You need to get the code from the provider (Sprint? whose SIM is it?). AT&T had a way to get the PUK code from their online account manager. If Sprint offers CDMA/GSM hybrid phones like the Imagio they might have a similar online utility. If you got the SIM from another carrier that is probably who you would need to contact for the PUK code.

Typically if you enter an incorrect PUK 10 consecutive times the SIM will become permanently locked and will have to be replaced.

One simple thing is to call up Verizon and ask them for it....they will gladly give it to you.

Or a temp fix would be to go to comm. settings and change from Global to CDMA...
