Missing APN settings screen

I have and HTC Imagio from Verizon and I've successfully unlocked the sim and changed network settings to make phone calls and text, but unfortunately Im missing a screen under settings>connections> then the advanced tab>new connections. After entering a new connection it is supposed to go to the APN screen but all I see is the dial out entry screen for CDMA.

Are you in cdma or gsm mode? I am assuming you are using a gsm sim card so you need to be in gsm mode.
