How to unroot and restore Evo back to stock

I have a White Sprint Evo.

It has Synergy Rom (RLS1) Android 2.3.3 Baseband 2.15.00.0808 PRI 2.15

I do not know how to restore fully to stock. I have to give it to Sprint to order me a replacement.

Google the stock 2.2 RUU, rename to PC36IMG.zip and put it on the root of your SD card. Reboot into HBOOT and answer yes when it asks you if you want to run the file.
