Please Help A Newb Out I F'd Up My Internet Connection

I dont even know what I did but somehow I got to a screen where it forced me to either choose remote access or to put in my IP address

Now Im having alot of problems connecting and my weather, etc wont update

Is there a way to change just my phone and modem settings to the default
settings??

btw I have a Samsung IP-830W on SPrint
