Phone dials last number by itself

This used to happen with my Mogul on some roms and now it's suddenly started happning on my Sprint TP. I was wondering if any one had this happen.

When I plug in the adapter in my car, the phone will occasionally dial the first number on the call list.

Occasionally
1. doesn't do it the moment I plug the adapter in
2. doesn't do it continuously every time.
3. sometimes when it starts doing it, it is unstoppable until I soft-reset
4. sometimes it will do it once or twice in a row then quit.

definately has to do with the audio adapter.
does not do it when I'm plugged in at home to the computer.

What's your audio adapter (what does it do and who makes it)? Maybe if someone else has the same equipment they can indicate if they've had the same problem. Sounds like a hardware problem or incompatibility. If it's some sort of car kit I could definitely see this happening as you might have an external interface to answer, reject, and redial/return calls with one-button interface, the same way double-tapping a BT headset button often redials (or something similar, depending on the headset).
