Camera problem

I was having a problem with the camera on my Sprint CDMA TP - I did a hard reset and am still having the same problem.
Every picture that I take is blurry except for pictures taken very close to the camera. It's almost as if it's in some sort of macro mode. I have changed no settings on the TP at all - as I said, even after a hard reset, if I open the camera app I have this problem.
Any ideas?

is the metering mode on average or center area...center area makes things a lot clearer

Is it focusing properly? This should be visible in the on-screen preview.

Metering is set to center and it's not focusing properly - pictures appear blurry on the screen except when very close to the subject.
