You should have received a receipt with the phone that has instructions for sending in the receipt plus the UPC off the box it came in. Don't forget to sign the receipt and check your return address for correctness . Cool?

My package only had a packing list and activation instructions....no mention of MIR.....anyone know of a website to print off the info for the MIR?

If this is for Sprint (I assume it was) they would have emailed you a receipt. On the receipt was a link to get the rebate form.

I didn't get anything either for the MIR so I just went to the touch pro 2 on the site, click on the MIR and printed it out
