Revert/Restore Sprint Diamond back to normal!!

I found a forum about a week ago that had the inital MIP settings and profile defaults to revert back too. I was wondering if anyone could send me the link or post the link. I have a Sprint Diamond that I had on the VZW Network that I accidentally de activated so its no good to me since they have closed the loop hole. If someone could point me in the right direction as to the initial settings that would be great! Thanks!
