Internet Sharing in Stealth Mode

From using other HTC WinMo devices, I've learned to install 'stealth' mode Internet Sharing cabs, for USB to PC Internet Sharing. Can I assume that is also required for the stock Diamond ROM, and if so, where can I find that cab?

No need to install a cab.
Just make these reg edits:

HKLM\Comm\InternetSharing (delete “extension”)
HKLM\Comm\InternetSharing\Settings
MaxCMConnectRetries = 0
ForceCellConnection = Sprint
HKLM\Software\OEM\WModem
CheckProfile = 0
Multi-NAI = 0
WModemDUN = 0
HKCU\Software\Microsoft\InternetSharing
LastCellConnection = Sprint

Then you'll be good!

Once this is done, does it matter how you launch the internet sharing?

Examples: checkbox when you first hook up the USB connection or the Internet Connection Sharing (ICS) program directly.

Just as wmdunn said, you can run ICS any way you want.
In the first edit you just delete the value or extension at that key.
BTW I did find a cab that another user made but along with those edits he put in several others to keep his device from going into low power mode which aren't necessary. Better to do the edits yourself. IMO

I'm putting this in a .reg file. It's all working except the key delete. I'm ending up with extension equal to '-' when the '-' is supposed to delete the value, I think. Here's my entries:

Well, actually you just need to navigate with your reg editor to:
HKLM\Comm\InternetSharing

Then click and hold down with your stylus on the value that shows up under internet sharing and just select delete or clear. It will probably ask you if you want to delete it and select yes.
I use and recommend PHM Reg Editor for doing the edits. It's free and available in the WinMo software forum here. Any regeditor should work though.
