How do you remove icons from WM 6.5?
 

Sometimes can't uninstall apps... or when you upgrade to new version, copy of old icon still lingers.

open your file explorer
go to "my device"
open "windows" (it'll take a sec, its a big folder)
open "start menu"
if the icon is in a folder select the appropriate folder otherwise all of you icons will now appear, click and hold the one you want deleted until the menu pops up and choose delete
