I know this thread is a little old, but I am having the same problem... I thought it was related to something I was installing, but its random. I've done a hard reset and installed apps until the problem would start. Then I would hard reset again and install the app that caused the problem and it would work fine. So I went ahead and installed my other apps while checking the today screen. It stopped updating the today screen changes after a different app was installed.

So I started over again and installed the first 2 apps that caused the problem with no problems. I would soft reset after each app installed before installing the next also. After a few more apps were installed the today screen stopped refreshing again.

Last night I finally got everything installed and the today screen was working correctly. So this morning after I get to work I removed a couple of plug ins from the today screen and the damn thing isn't updating again... I've let it sit to see if maybe it would update after a while, but its been sitting for 3 hours now and will not update. It will update if I soft reset. This is starting to piss me off.

Heres what I have installed in the order I installed it last time.

OEM Sprint 2.16 Rom
No registry hacks
Resco explorer 2007 with FTP, Registry, and today plugin add in.
SPB backup
MemMaid 2.0
HTC home
Emunutz HHC
HTC Audio Manager
CodyPPC Threaded SMS
Arcsoft MMS
HTC X-Button
Remote Desktop
TCPMP
Flash Bundle
Oxios Memory

The problem happened the first time after HTC Home was installed, the second time I installed Windows Live Mobile, and the third time happened after I installed the threaded SMS app.

Anyone else run into this recently and maybe have a fix... It looks like I'm heading to Sprint this afternoon...
