It may not be exactly what you had in mind, but I bet this would work for you...
Touch InCall Screen Tweak -- It has an 'always on' option that might do what you need. If nothing else, you could just tilt the phone briefly vertical than back into hand-held position if it did go to sleep on you...no need to push any buttons.
